What other documents are needed to join?

There are a couple of other documents we need to hold on file here for when we are organising a licence for your child to take part in performance related work (*see applying for a Licence in frequently asked questions). Without these documents held at the office for you child we are unable to submit your child for work.

• Doctors Note
• You will be required to send us a Doctor’s note which will state your child is in good health and able to take part in TV or Photographic work. This will be valid for six months from the date on the Doctor's headed note paper. This is a standard note you can ask your GP to supply or you can download a Doctor's Declaration Form from the website and ask your GP to sign. Please make a note somewhere safe to have this renewed every 6 months.
• Two Passport Photos.
• Copy of your child’s birth Certificate
• Part II Licence Application (this can be downloaded from our website)
